Patient Forms
NEW PATIENTS
New patients can save time during their first appointment by completing the Patient Registration form prior to their visit. Simply print out the Patient Registration form, fill in the information requested, and bring the completed form with you to your appointment.

Cancellation Policy
$25 FEE FOR RESCHEDULING WITHIN 24 HOURS OR NO SHOW
A scheduled appointment has to be cancelled at least 24 hours prior to the appointment time or a $25 fee will be charged. Please note the date, time of your call to cancel the appointment and also the name of staff person, you had spoken to.

Insurance
PLEASE CONFIRM WITH YOUR INSURANCE CARRIER
All major insurances, both government and private, are accepted. It is best to confirm this by calling your insurance carrier, or you can fax or text the front and back of your insurance card(s) and our staff will investigate this for you.

Medication Refills
ASK THE PHARMACY REQUEST AT 910-609-1836 OR 919-363-7572
Patient should ask the pharmacy to send a faxed request for refill. Patient can call for medication refills during normal office hours, leave a voicemail message after hours noting the date and time when they left this message.
Medical Records
MUST SUBMIT A SIGNED REQUEST
A patient or their representative must submit a signed request for medical record request release. There will be a charge for medical records based on the length of the medical records. Due to the high volume of requests for medical records, it can take up to 30 days to fulfill your request. Please call office for further details.

FOR THE USE OF MODERN DEVICES LIKE INSULIN PUMPS, GLUCOSE SENSORS, BLOOD GLUCOSE METERS, ETC.
Our clinic has participated in the evaluation of new endocrine medications like insulin therapies and other medications with major drug manufacturers like Lilly, Novo Nordisk, Glexosmith Kline, Sanofi, Roche Diagnostics to mention a few. We are available for international consultation with our overseas clients, who may be interested in getting help in clinical management and set up of endocrinology services at their clinic or hospital for the use of modern medication and devices like insulin pumps, blood glucose meters, and continuous glucose monitors.
